***In consideration of the acceptance of my membership in the :Kaizen Futbol P.M.A., Private Training, I, the Participant and/or Parent/Guardian (if
Participant is under 18 years of age), agrees as follows:

Consent for use of Personal Information:
I authorize :Kaizen Futbol P.M.A. (:K.F. P.M.A.), to collect, use and disclosure of personal information about me and/or the Participant for the purpose of receiving communications and the purposes described herein within :K.F.P.M.A. I understand that I may withdraw consent to collect, use or disclosure of my and/or my child’s personal information at any time by contacting Kaizenfutbolpma@gmail.com.*We do not sell or distribute your personal information to any other third party not listed herein and within :K.F.P.M.A.

Photo, Audio and Video Release
I grant permission to :Kaizen Futbol P.M.A. and its representatives the irrevocable right and unrestricted permission to photograph and/or video, training sessions of the Participant/ or my child while participating in :K.F.P.M.A. training sessions and/or record my and/ or the Participant’s image and/or voice on still or motion picture film and/or audio tape, and to use this material to promote futbol/soccer or :Kaizen Futbol P.M.A. and Private Training, through any or all of the following formats, including but not limited to the media of website, flyers, brochures, newsletters, presentations, social media, editorial publications, catalog, advertising use and other marketing activities. This grant includes the right to modify and retouch the images at the discretion of :K.F.P.M.A. of newsletters, websites, television, film, radio, print and/or display form and waive any claim to remuneration for use of audio/visual materials used for these purposes. I am fully and without limitation releasing :K.F.P.M.A. from any liability that may arise from the use of the photo images, audio or videos.

I Understand that I, /or my child cannot participate in any training sessions until after registration has been completed; data collected, entered and validated in the :Kaizen Futbol P.M.A. registration database.

Acceptance of Terms and Conditions
I have reviewed the :Kaizen Futbol P.M.A. Training Registration Form agreement and my signature affixed to hereto indicates my agreement.
I accept sole responsibility for my or my child’s personal possessions and training equipment.
I am aware of :Kaizen Futbol P.M.A. Private Training sessions Motto ‘Attitude is Everything’, ‘Always Improvement’, ATPMA, 5P’s.
I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators and representatives.
